Change of address: If you have recently moved or if the address on your pay stub is incorrect, please fill out a change of address form to update your address information.

W-2 Request form: Request a W-2 from Maxim’s payroll department. Or, for more W-2 information call the W-2 hotline number at 1-866-405-1107.

W-4: Download a .pdf version of the W-4 form.

W-5: Download a .pdf version of the W-5 form.

Direct deposit authorization: Download a direct deposit authorization form. Please submit the completed form with a voided check or printed deposit slip directly to the office.
