About Maxim's Transitional Care Program: Maxim’s Transitional Care Plan main objective is to reduce
the chance of our clients being readmitted to the hospital after 30 days of
discharge. It empowers patients to take charge of their healthcare. We build a relationship with the patient while
in the hospital and develop a custom care plan that addresses patient-specific
factors both in the hospital and post-acute settings.
